Your Medicare card with valid expiry date needs to be filled in on this sheet. Email this back to us at reception@careywang.com so that we can give you an appointment.

You must wear a mask to come to the office. None will be provided. You will not be allowed entry without one.
Only you will be allowed into the clinic. NO EXCEPTIONS. Anyone accompanying you will have to wait for you outside the clinic.
